To apply to the University of North Dakota Physician Assistant Program for the class beginning in May 2010, you must apply through Central Application Service for Physician Assistants (CASPA)

The application process is two fold.  You must:

• Complete the CASPA primary on-line application by going to: https://portal.caspaonline.org/
• Complete supplemental forms that include:
  • University of North Dakota Graduate School application at: Graduate School Application
  • Have your proposed physician preceptor complete the "Preceptor Profile".  This document is available in two different formats: Preceptor profile_revised_2_09 or Preceptor profile_revised_2_09                                                                                        (Word 2007 format)                             (Adobe PDF file)                                                                   Preceptor Profile: This form should be downloaded from this website and given to your proposed preceptor to complete.  Ask the preceptor to return the completed form to you in a sealed envelope.  You must then forward it to the University of North Dakota Physician Assistant Program at the address below and have it postmarked no later than September 1st, 2009.
