A member of NORTHSTAR is any primary care provider who is interested in becoming more active in research and quality improvement.

How to become a NORTHSTAR member

Go ahead and complete the NORTHSTAR enrollment form and return it to our Network Coordinator, Jessica Behm, via e-mail, fax, or mailing. Membership is free and begins once we receive your form.
