Objectives of the Elective:

  1. Priority sound approach to developing a history and physical examination at the level of the specialist in Internal Medicine and using this to draw concise problem list.
  2. To develop a consistent and continuous approach in the use of a problem list and plan of evaluation of treatment to return the patient to good health at the least possible cost as quickly as possible.
  3. To learn how to communicate with the patient at the level of patient's sophistication as well as develop good communication and rapport with the allied health personnel involved.
  4. To learn essential procedures such as LP, thoracentesis, paracentesis, bone marrow biopsy, endotracheal intubation, and central line insertion. For those procedures that are not actually done, video tapes may be available for review as well as literature describing them.
  5. To cover as many medical problem areas as possible during the four week rotation and develop a sound approach to their evaluation.
  6. Develop an approach to preventative medicine; i.e., modification of coronary risk factors, dietary approach, early detection and prevention of cancer, etc.

Instructional Activities:

  1. Performance of a complete history and physical examination developing a problem list and plan of approach. Dictation and case presentations will be included.
  2. Procedural skills to be taught by an experienced physician. Videotapes or literature made available if the procedures are not encountered during the rotation.
  3. Direct patient management including daily examination, interaction with patient and/or family as well as other physicians and allied health professionals. This also includes documentation on progress notes, procedure notes, discharge summaries, etc. If dictation available, instruction on appropriate use of dictation equipment.
  4. Didactic sessions with preceptor physician covering topics concerning the medical care of patients, etc.

Standards of Performance (Outcomes) Indicating Attainment of Objectives:

  1. Demonstration of performance, presentation and dictation of history and physical examination including problem list and plan of approach to the attending physician at the level of an internist.
  2. Demonstrate learning and improvement of procedural skills. Instructed during the rotation to the attending physician.
  3. Demonstrate initiative and direction in leading the medical team in the care of patients.
  4. Demonstrate appropriate documentation either by hand written progress notes that are clearly legible or dictated notes, exams, summaries, etc.
  5. Adequate formal presentation (topic decided upon between preceptor and student) to physician preceptor or other group of physicians.

Criteria for Grading:

  1. Rapport, communication skills, physical appearance, and empathy demonstrated during rotation. This includes patient interaction as well as interaction with staff and allied health professionals.
  2. The development of appropriate history and physical examinations including problem list, plans and their documentation. The demonstration of the student's analytical abilities and the ability to carry out plans. The above will be done with adequate use and knowledge of appropriate references.
  3. Adequate demonstration of learning and improvement of procedural skills.
  4. Awareness of preventative medicine including cost, modification of risk factors, diet, and early detection of disease.
