Elective Title:          International/Developing Nation Medicine Elective

 

Campus:                   All Campuses               Location of Elective:  Chimbote, Peru

 

Preceptors:               Stephen Tinguely, M.D., Napolean Espejo, M.D.,

and Mark Christianson, M.D.

 

Periods offered: One period annually to be determined by the preceptors.   For academic year 2008-2009 the elective will be offered during Period 5, October 20-November 14, 2008.  Student must contact Dr. Stephen Tinguely prior to selecting this elective.

 

Number of Students:   To be determined annually by Dr. Stephen Tinguely

 

Purpose:  To gain experience in the unique challenges and strategies of health care delivery for people living in a developing country.

 

Unique responsibilities of elective:

 

1.         Expenses:  The student is responsible for all travel expenses, meals, personal safety and health. 

2.         Immunizations: The student is ultimately responsible for making travel arrangements.  The preceptor will assist in coordinating travel planning.

3.         Travel arrangements: To be made by the team coordinator at least two months prior to the anticipated dates of travel.  The student is expected to get all the requested information to the team coordinator at the scheduled times.

4.         Liability: As a student in good standing at the UNDSOMHS, medical liability is covered by UND.  Personal liability is the responsibility of the individual student.

5.         Travel insurance: The student is responsible for travel insurance.

 

Objectives:  Following successful completion of this elective, the student will be able to:

 

1.         Evaluate health care needs among a population of underserved and impoverished people residing in a developing country.

2.         Report on established methods of providing basic health care by local health care providers and compare to health care delivered in developed countries.

3.         Perform and report a health care needs assessment survey and evaluation at the location of the elective.

4.         Construct an implementation plan to meet one practical local health care need at the location of the elective.

5.         Deliver culturally-sensitive primary health care to an indigent population under
            the supervision of local health care providers.

 

Instructional Activities: During this elective, students will be involved in/experience:

 

1.         The formulation of personal learning objectives at outset of the elective.

2.         Primary health care delivery within neighborhood medical posts under the supervision of local health care providers.

 

 

3.         Current and/or previously established public health care delivery projects (examples might include sanitation evaluations, fluoride varnish administration, vaccine administration, in home medication administration, hospice care etc).

4.         The delivery of medical care and performance of procedures by visiting US physicians (examples might include GYN surgeries performed by MeritCare physician/nurse teams).

5.         A needs assessment survey design and data evaluation.

6.         An action plan formulation.

7.         An action plan implementation of previous year’s needs assessment.

8.         An evaluation of previous year’s intervention.

9.         Personal journal writing.

 

Criteria for Grading:  During and following this elective, the preceptor will:

 

1.         Complete the standard UND 4th year evaluation form with particular attention to the skills obtained in     the items listed above under “Criteria for Grading”.

2.         Assess the quality of student’s needs assessment survey and action plan and discuss it with the student.

3.         Request feedback from local parish/community health care leaders regarding the quality of the student’s participation.

4.         Evaluate the extent of the success of meeting the student’s own learning objectives for this elective.

5.         Judge by direct observation of clinical skills and review of the patient medical records the student’s ability to perform an efficient history and physical exam on the patients.

6.         Judge by direct observation the student’s level of proficiency in diagnosis and treatment of the various diseases/conditions encountered at the elective site.
