Physical Therapist Assistant Licensure by Endorsement:

 

For those physical therapist assistants interested in applying for license by endorsement to practice in the State of North Dakota, the following information is required:

1.      Complete, have notarized & return the “Application for Licensure as a Physical Therapist Assistant” form, including a photograph.

2.      Enclose a check for $150 for the non-refundable application fee.  Please make check payable to NDSEC for PT.

3.      Complete & return the Juris Prudence Examination.

4.      Contact the institution granting your physical therapist assistant degree & have an official transcript, indicating graduation from an accredited program, sent directly to the Committee.

5.      Enclose a copy of current registration(s).

1.      Send a verification of license form to each of the states/provinces in which a license is held/previously held.  Please request that this completed form be sent directly to the Committee.

2.      Contact the FSBPT (Federation of State Boards of Physical Therapy) to have your NPTE (National Physical Therapy Examination) scores forwarded directly to North Dakota. 

3.      Enclose letters from department heads/personnel officers from the last two places of employment, verifying employment.

Eligibility for permanent licensure is possible upon receipt & review of all required documents & verification of license(s).

 

Enclosed is an explanation of continuing education requirements which will be effective upon receipt of a permanent license.

 

If you have not been employed as a physical therapist assistant during the past 5 years or have not passed the National Physical Therapy Examination (NPTE), the NPTE must be taken.
