Instructions for ND licensure as a Physical Therapist

(for applicants who would like to reinstate their ND license) 

1. Download PT Application Form in MS Word Format or PDF Format
    

2. Complete the Application Form:

   • Make sure you sign the form and have it notarized 

   • Include the ND application fee of $150 made out to NDSEC PT (may be personal check or money order)

   • Include a copy of current license(s)

   • Include a letter from department head/personnel officer of the last two places of employment, verifying employment.

   • Download a verification of license form and send to all jurisdictions in which you have been licensed

   • Return the ND Application Form and fee to the NDSEC PT
      

3. Download the North Dakota Juris Prudence Examination:

   • This is an open book examination, you can download or view the ND Rules and Regulations and the ND Practice Act

   • Complete and sign the examination form

   • Return the form to the NDSEC PT Office

If you are unable to download the above forms, please contact our office. 

 

Eligibility for a permanent license is possible when all of the above requested information has been received. 

 

If not employed as a physical therapist during the past 5 years or have not passed the National Physical Therapy Examination (NPTE), the NPTE will have to be retaken.
