Trained Physical Therapist Licensure by Examination Information:
The requirements for foreign educated physical therapist
interested in applying for licensure with
1. Complete, have notarized & return the “Application for Licensure as a Physical Therapist” form.
2. Enclose a check for $150, payable to NDSECPT, for the non-refundable application fee.
3. Complete & return the Juris Prudence Examination.
4. An evaluation of credentials must be sent directly from the FCCPT (Foreign Credentialing Commission on Physical Therapy) indicating requirements have been met for a Type 1 Certificate. For information on the educational, language and prior licensure requirements, please contact the website www.fccpt.org
5. Application for the NPTE must be done online through the website www.fsbpt.net/pt
6. Enclose letters from department heads/personnel officers from the last two places of employment, if applicable, verifying employment.
The application for licensure form and the juris prudence examination can be downloaded from the website www.medicine.nodak.edu/pt under Physical Therapy, Licensure. You may also find the Practice Act and Rules/Regulations and an explanation of continuing education requirements there. If you are unable to download this information, please contact our office.
The above information must
be received prior to the Committee authorizing the FSPBT to allow you to sit
for the NPTE.
Please refer to the FSBPT website at www.fsbpt.org for the NPTE Handbook, school codes, and other examination information.
Upon receipt of the above information and a passing score on
the NPTE, you may be eligible for licensure as a physical therapist. After the Committee is notified of the name
