North Dakota's Coordinated School Health Program

Graphic of the Model I Program CHPPR provided consultation and technical grant writing assistance to the Department of Public Instruction in preparing a CDC-sponsored grant proposal to support a Coordinated School Health Program (CSHP) aimed at reducing chronic disease risks. This grant was funded in March 2003 and CHPPR assisted the North Dakota CSHP staff in developing logic models for program planning and activities. In addition, CHPPR provided the evaluation service component of the CSHP. CHPPR evaluated the CSHP staff's efforts, activities, and accomplishments over a five-year period. The goal of the CDC-supported Coordinated School Health Program in North Dakota was to improve the health status and cognitive performance of North Dakota youth by reducing tobacco use, improving diet, increasing physical activity, and decreasing prevalence of HIV.
